Welcome, New Students!
We are so glad you are about to become a FAIRVIEW EAGLE! We take great pride in that and hope you will, too. We think you'll find Fairview School to be an exciting and friendly school.
Parents will need to come to Fairview School to enroll their child in school. They will complete some contact information and information about their child’s previous school. We will request records from the previous school, assign a new homeroom teacher, and give the new student a “buddy” to help them find places during the first week they are at our school. We will also need to know about bus or car transportation to and from school. If the student is enrolling in kindergarten and has not been in another public school, we will also need a birth certificate and immunization record.
Before You Enroll
Please make sure you have:
- Proof of residence (utility bill, driver's license, etc.)
- Current immunization record
- Current report card or transcript from previous school
- Special needs documents if applicable (IEP, for example)
- Legal guardianship papers if applicable
- Birth certificate
- Social Security card
- Phone or FAX number for previous school
- Valid photo ID for parent or guardian
Home Schoolers: Please bring as many records as possible.
Where to Go
Come to the Front Office located in the lobby of the school's main entrance.
Parent or guardian you must be with your enrolling student.
Once You Arrive
At the office, you will be given an enrollment packet containing forms that must be filled out before enrollment can take place. These forms include:
- Demographic Form
- Home Language Survey
- Food Program Form
- Registration Form
- Health Information Form
- Parent/Guardian Affidavit
IMPORTANT: You are welcome to pick this packet up ahead of time to fill out at your leisure if you would prefer. The Parent/Guardian Affidavit must be filled out in the Office after all of the other forms have been completed.
We do suggest you register at least the day before your student plans to begin school so that teacher assignments/schedules can be made.
